Application Form for Production Assistance

Please fill out the following form and submit it to the Osaka Film Council. Scenarios and other related materials are also appreciated.
Please abide by the following conditions upon application

Osaka Film Council, upon requests from filmmakers, offers information on possible locations and acts as an agent to gain shooting permissions from property owners. The Council, however is not responsible for concluding any contracts, making any payment, nor compensating for any damages. The filmmakers themselves shall be responsible for these matters.
The name of "Osaka Film Council" shall appear on the end credit. The same applies to other locations sites (upon agreement of each property owners).
The filmmakers shall submit to the Council photographs of shooting scenes (including the location site, staffs and cameras).
